Working in close collaboration with Children’s Hospital Primary Care Center (CHPCC) Behavioral Health (BH) clinical and administrative teams, the Senior Resource Specialist manages the administrative processes to connect CHPCC patients with BH services. This is a key position helping to support various innovative behavioral health initiatives.

The Senior Resource Specialist shall be responsible for:

  • Facilitating, tracking, and following up on behavioral health (BH) referrals (both within and outside of BCH), in close collaboration with primary care BH clinicians and administrative staff.
  • Connecting with patients and families to provide support and guidance around the BH referral process, as well as implementing ongoing care plans.
  • Collaborating with other BH team members to track and monitor referrals, in order to help ensure continuity of care and mitigate risk.
  • Assisting in the coordination of BH case management functions.
  • Acting as a liaison with internal BCH and community resources and services.
  • Advocating on behalf of families to help ensure timely access to services.
  • Initiating, developing, and maintaining relationships for accessing community-based resources.
  • Engaging in outreach and maintaining positive working relationships with interdisciplinary primary care staff (including physicians, nurses, BH clinicians, and administrative staff).
  • Completing documentation of work in the electronic medical record.
  • Developing and maintaining databases regarding patient referrals and BH referral resources.
  • Assisting with tracking outcomes and supporting program development and strategic planning processes, analyzing data and summarizing findings, and providing information to support community partnerships and grants. May recommend program or procedural changes to improve outcomes or efficiency.
  • Reporting status and outcomes of referrals to referral source.
  • Coordinating logistics for plan adherence such as appointment reminders and arranging transportation.
  • Explaining hospital/community-based resources to patients and families.
  • Attending and participating in case review and care planning meetings.
  • Serving as a subject matter expert to Patient Navigators, Interns, Social Workers, Psychologists, and others on BH resources and specialized services offered in the clinic.

To qualify, you must have:

  • High School / GED
  • Minimum 2 years of experience in a behavioral health environment.
  • Ability to effectively coordinate discharge activities within the assigned unit/team.
  • Intermediate assessment and problem-solving skills.
  • Intermediate knowledge of medical terminology and experience with Electronic Medical Record.
  • Understanding of behavioral health continuum of care.
  • Participate in a department or hospital wide committee or project in a leadership/SME capacity.
  • Ability to mentor and teach other Resource Specialists about the role.
  • Possess excellent communication skills to work with different teams.
  • Demonstrate planning, prioritizing & time management skills to ensure that patient needs and timelines are met
