The Patient and Family Educator is responsible for the following:

  • Identifies, develops, integrates, and rolls out applicable resources and programs for the patient/family education program.
  • Establishes rapport, supportive and ongoing relationships with patients and their families from a variety of cultural, ethnic, and religious backgrounds.
  • Assists leadership in the formulation and achievement of goals for the patient and family education program.
  • Review medical notes and physical assessments to identify asthma symptoms and management trends between the hospital, home visits, and family reports for plan development.
  • Frequent assistance with educational consulting to parents and community members regarding asthma management.
  • Frequent consults with Pulmonary, Allergy, Primary care providers, Nurse practitioners, Social Workers, and Nurses.
  • Education-related resources and guidance (e.g., home-schooling, online education, advocacy, tutors, missed school days, etc.).
  • Referrals to community resources.
  • Triaging referrals from PowerChart & Email.
  • Occasional public speaking engagements.
  • Writing, collaborating, and consulting on letters with Providers for services/supports within the school environment, using appropriate language and terminology, most specifically for an asthma-friendly environment.3
  • Meet with patients and families in the home setting to improve exposure to environmental triggers.
  • Works collaboratively and functions as an effective member of the health care team as well as a contact within the hospital and community.
  • Seeks opportunities to foster professional growth of self and others.

The Patient and Family Educator must have the following:

  • Bachelors degree required/Masters preferred.
  • One year of relevant experience required.
