40-hour 100% onsite social work position to provide a range of clinical social work services and case management to patients in the lung, intestine and liver transplant program. This social worker will care for the organ group population in all phases of transplant (evaluation, transplant admission and follow up).
Key Responsibilities:
- Psychosocial assessment as part of transplant candidacy for the program
- Counseling and crisis intervention
- Facilitation of the use of hospital and community services
- Resource information and referral, and program development
- Psychosocial consultation to multidisciplinary team
Minimum Qualifications
Education:
- Master’s degree in social work
Experience:
- 2 years plus experience as a licensed Social Worker
- Experience in medical environment, bereavement, child protection issues preferred
- Flexibility and ability to work in a fast-paced, dynamic environment
- Excellent communication skills are necessary.

Licensure/Certifications:
- LICSW Licensure in state of MA
This position is 100% onsite
