Department/Position Summary:
CARES for Kids is a MassHealth program to support clinics that care for children with medical complexity. This program will be available for solid organ transplant patients in the Pediatric Transplant Center (PTC) who are MassHealth members under 21 years old who meet the medical necessity criteria. This RN will be responsible for leading the development, implementation and oversight of the coordination for Boston Children’s Hospital CARES for Kids Program within the PTC. The RN will serve as the clinical expert and strategic resource to get eligible transplant patients enrolled in the program, work with the centralized CARES program at BCH and coordinate with patients and families.

Key responsibilities:

  • Implementing, overseeing, enrolling, documenting and coordinating the CARES for Kids Program for the PTC
  • Partnering with the BCH centralized CARES for Kids staff to support, enroll and be compliant with the program’s requirements.
  • Providing staff education and development programs for the CARES program
  • Working collaboratively with multidisciplinary team of MDs, NPs, RNs, Administrative, Social Work, Pharmacy and Registered Dietician colleagues to provide comprehensive care to pediatric patients with lung failure and those pre and post lung transplant.
  • Pre-clinic preparation, clinic session support, phone triage, patient/family education and longitudinal care coordination in the lung transplant program.
  • Provide cross coverage RN needs across all PTC programs
  • Working 40 hours per week Monday through Friday, four 10 hour days.

Minimum qualifications:
Education:

  • Bachelor’s of Science Nursing Degree.

Experience:

  • A minimum of 3-5 years of acute care RN experience; pediatric and transplant experience preferred.

License/certification:

  • A current MA RN License
  • BLS certification

The posted pay range is Boston Children’s reasonable and good-faith expectation for this pay at the time of posting.
