Position Summary:
This position will support clinical teams, patients, and families with discharge planning and care coordination needs for Boston Children’s Inpatient Psychiatry and Community Based Acute Treatment (CBAT) programs.
Key Responsibilities:

  • Engage with patients and caregivers in-person or by phone to assess health-related social needs such as housing, transportation, food access, and financial support.
  • Connect families with appropriate internal and community resources, including referrals and service navigation.
  • Serve as a liaison by building and maintaining relationships with healthcare staff and community agencies to improve service access.
  • Provide care coordination and support to help families overcome logistical barriers to receiving medical care.
  • Document all interactions and interventions accurately in the electronic medical record (EMR) system.
  • Collaborate with multidisciplinary teams, contribute to program development and quality improvement initiatives, and participate in regular supervision and team meetings.

Minimum Qualifications
Education:

  • High School diploma/GED required, bachelor’s degree in social work referred

Experience:

  • 1 years of prior experience working in community agencies and/or a medical setting required
